Contract Employment Experiences of Visiting Nurses at Public Health Centers in the Metropolitan Area: Focused on Employment Type and Treatment

- Purpose
This study was conducted with a focus group interview that drew out experiences, limitations, and difficulties in the workplace according to the employment conditions of visiting nurses in the public health centers.
Methods A total of 12 visiting nurses are those working in the public sector in Seoul and Gyeonggi province who were willing to participate in the interview. Analysis categories and coding were divided into three categories: compensation system, occupational status, and opinions to improve their treatment. Using the content analysis method, the current working status and compensation system of visiting nurses were described.
Results The main themes derived from the significant statements of visiting nurses were ‘Ten years of frozen salary system’, ‘Full-time workers of their own league’, ‘Excluded from performance benefits’, ‘Every visiting nurses are virtually precarious’, ‘Experience of exclusion and discrimination’, and ‘Reasons and barriers to be a full-time worker’. All of the visiting nurses working in the community insisted on having equal treatment for work of equal value. Visiting nurses in the public health sector wanted to be set to the same payment system and the fair allowance system as well. It is necessary to continuously seek solutions to the problems left in insisting on the civil service of visiting nurses.
Conclusion Visiting nurses who were working in a precarious job position felt job insecurity, and experienced discrimination, alienation, and exclusion. Legal and institutional reform is needed to improve the treatment of visiting nurses.

- Purpose
A care coordinator is an emerging nursing professional role in South Korea. The purpose of this study was to identify educational needs and priorities for care coordinators among nurses.
Methods An online survey was conducted on 661 current or retired nurses from January 30 to February 28, 2021. A total of 17 essential competencies for care coordinators, recognized based on literature review, were used to analyze the educational needs. The data were analyzed using descriptive statistics, a paired t-test, and one-way analysis of variance with SPSS 25.0. The educational needs analysis was conducted by using a paired t-test, the Borich Needs Assessment Model, and the Locus for Focus Model.
Results Five contents were identified as the first priorities for educational needs: ‘Health program planning and evaluation’, ‘Care planning’, ‘Coordinating community-based services’, ‘Case management’, and ‘Transitional care’. The second priorities for educational needs included 'Population health management' and'Welfare resource linkages via communicating with social workers’.
Conclusion The priority items derived from this study offer underpinning insights for the development of care coordination training program.

- Purpose
This study was to review the previous studies on the ‘Willingness to Pay (WTP)' for healthcare services and suggest future implications for nursing research. Methods Using the scoping review method, we used RISS, KISS, KMbase, Koreamed, PubMed, EMbase, CINAHL as searching engines. According to the selection and exclusion criteria, 40 appropriate studies were selected and analyzed. Results 24 studies were categorized into medical service field among medical, public health, and nursing service fields. A total of 16 studies were related to healthcare system (policies), 13 studies were to the healthcare intervention, and 11 studies were categorized into the health management. Most of the methods for eliciting WTP (70%) were about a contingent valuation method (CVM), and the use of double bounded dichotomous choice (DBDC) tended to increase. In the nursing field, five WTP studies were identified: two studies published in the early years of 2000, which were conducted on hospital-based home health visit services. Recent studies were mostly about counseling and education by advanced practice nurses (APNs). Conclusion WTP studies on healthcare services were largely published from the medical fields and health policy areas with the CVM method. In the field of nursing, studies have been conducted on the subject of limited service areas. More active exploration of research topics is required, particularly under the current policy setting, where discussion of the public health insurance fee for nursing practice is essential.

- PURPOSE
The aim of this study is to identify core keyword of frailty research in the past 35 years to understand the structure of knowledge of frailty. METHODS 10,367 frailty articles published between 1981 and April 2016 were retrieved from Web of Science. Keywords from these articles were extracted using Bibexcel and social network analysis was conducted with the occurrence network using NetMiner program. RESULTS The top five keywords with a high frequency of occurrence include ‘disability’, ‘nursing home’, ‘sarcopenia’, ‘exercise’, and ‘dementia’. Keywords were classified by subheadings of MeSH and the majority of them were included under the healthcare and physical dimensions. The degree centralities of the keywords were arranged in the order of ‘long term care’ (0.55), ‘gait’ (0.42), ‘physical activity’ (0.42), ‘quality of life’ (0.42), and ‘physical performance’ (0.38). The betweenness centralities of the keywords were listed in the order of depression’ (0.32), ‘quality of life’ (0.28), ‘home care’ (0.28), ‘geriatric assessment’ (0.28), and ‘fall’ (0.27). The cluster analysis shows that the frailty research field is divided into seven clusters: aging, sarcopenia, inflammation, mortality, frailty index, older people, and physical activity. CONCLUSION After reviewing previous research in the 35 years, it has been found that only physical frailty and frailty related to medicine have been emphasized. Further research in psychological, cognitive, social, and environmental frailty is needed to understand frailty in a multifaceted and integrative manner.

- PURPOSE
This study is to investigate working conditions including job stress among visiting nurses in public health centers in Korea. METHODS An social network based mobile survey was conducted in May 2017 (N=936, response rate: 47.0%). RESULTS The visiting nurses in this study had their average total career as a nurse is 13.7 years. The 68.3% of them were employed in an indefinite term, 17.0% were hired in a fixed term, and 11.0% came from outsourcing. They responded as high job-stress level including inadequate compensation (71.22/100) and job demands (71.91/100). They experienced down-talk (63.4%), swearwords (32.9%), being made a dirty face (39.9%), sexual jokes (30.8%), or being likened or evaluated with their appearance sexually (14.3%). Among the causes of job related conflicts and discrimination, deprived salary level was the most frequent reason (83.4%). The conflicts and discrimination were incurred by government officers (52.4%). There were no significant differences in overall job stress, emotional labor, organizational commitment, violence, and discrimination experience based on their employment types. CONCLUSION The differences in working conditions among the non-regular nurses were trivial, and their overall working conditions were poor. It is necessary to improve non-regular nurses' working conditions in order to make up the limitations of the Korean healthcare system which is centered on hospitals.

- PURPOSE
This study was to examine the clinical characteristics and mortality risk factors of geriatric hospital patients who visited one region-wide emergency department (ED). It's basically meant to develop criteria for the patient management of geriatric hospitals and to provide related information. METHODS A retrospective research study was implemented using electronic medical records. The subjects in this study included 484 geriatric hospital patients who were selected from among 15,994 patients that visited one region-wide ED between January 1, 2014, and December 31, 2015. RESULTS There were significant differences in death, a change for the better and no change for the better, which were results of treatment, according to the length of stay in hospital, hospitalization in a general ward, not having an operation or surgery, the presence or absence of malignant neoplasm, the insertion of foley catheter, intubation, ventilator and the insertion of central venous catheter. CONCLUSION The results of this study suggest that a patient management system is necessary in geriatric hospitals and that competent healthcare workers who can properly respond to emergencies are required as well.
