The Effect of Case Management Program for Diabetic Patients in Korean Community

BACKGROUND
We first launched the case management program for diabetic patients who were registered in NHIC (National Health Insurance Corporation) in Korea and conducted this study to assess the effectiveness of the program.
METHODS
During the period from October in 2002 to March in 2003, 30 case managers performed the program for 71 diabetic patients. We evaluated the effectiveness of the program based on the results of fasting blood sugar level, two hour post-prandial blood sugar level, knowledge for diabetes, difficulty index in diabetes management, and the health risk factor changes of them. We analyzed results related to these factors through chi2 test and paired t-test.
RESULTS
The average age of the subjects was 58.9, and the numbers of women and men were 43 and 28 respectively. The fasting blood sugar level and two hour post-prandial blood sugar level decreased from 164.3mg/dl to 146.5mg/dl and from 224.0mg/dl to 203.0mg/dl respectively. The knowledge for diabetes and difficulty index in diabetes management changed from 8.13 to 9.10 and from 3.52 to 2.91 respectively, and these changes were the positive. We observed improvement in self-test of sugar level, foot management, oral hygiene and proper medication but not in self-test of nutritional management.
CONCLUSIONS
This study revealed that the case management program for diabetic patients who were registered in NHIC is significantly effective. However, the program need to study further to understand its long-term effects.
Nursing Case Management Program Development for the Elderly at Hospital

PURPOSE
This study was performed to develop the case management program that can provide the qualitative health care service to satisfy the increasing needs for health care elderly and prevent the increase in medical cost cased by prolonged and repetitive hospitalization.
METHOD
The survey was completed as follows. First, 290 elderly patients who would be soon discharged in 4 hospitals are participated in the survey on needs of nursing care and case management programs. Second, 70 professionals working at 5 hospitals including doctors, nurses, social workers and so on are participated in the survey on the case management team operation.
RESULT
Level of needs to Nursing care and case management programs showed so high. Professionals suggested on the case management team operation. Finally, based on the survey and literature review, the case management program was developed.
CONCLUSION
In this study, the specific models for the five case management services were suggested. It is thought that the service provision by the case management team based on the multi-disciplinary case management program model suggested in this study would be suggested as a constituent of the effective health care delivery system for the hospitalized elderly patients.
A Study on the Job Satisfaction of Visiting Nurses of Public Health Centers in Korea

The purpose of this study is to identify job satisfaction and relative factors of job satisfaction in visiting nurses in public health centers. Data were collected by a self-administered questionnaire from May 1 to June 25, 1997. The subjects were 384 visiting nurses of public health centers in Korea. The results of this study were as follows; 1. Job satisfaction of Visiting Nurses was measured by a 5 point rating scale, the whole mean score was 3.08. The level of job prestige was highest among the six components of job satisfaction. The mean score of job perception was 4.05. 2. The factor affecting job satisfaction were job status, educational level and the number of visits home a month. 3. Factors affecting job perception were whether or not they had other certificates, whether they were educated about visiting nursing, and the status of these nurses. 4. Job perception and Job satisfaction had a significant correlation.
